  • Adoption Form

  • Welcome to ANIMALS LEBANON adoption program. We request the following information to assist you in selecting the animal you want to adopt. Upon submitting this form, you will receive contact information to set up a visit to the shelter in order to choose the most suitable companion for you. The next step would be a home visit to make sure the animal is placed in a suitable environment.
  • Please note,
  • - Adopters are asked to make a $40 donation for each adoption. This is used to cover veterinary care and boarding expenses of the rescued animal.
    - All our animals are neutered. This decreases health problem possibilities later on, as well as ensuring there are no more unwanted births.
    - Cats and dogs live up to 15 years. Be prepared to take care of this pet for the next 10-15 years.
    - Pets need annual vaccines and frequent check-ups. Be prepared for the ongoing responsibility and expenses of food and vet visits.
The prospective adopter is the one who should fill out the adoption form.
To be considered an adopter, you must be 18 years of age or older.
